DMHAS Housing Assistance Fund (HAF)
The DMHAS Housing Assistance Fund (HAF) grant provides funding to assist chronically homeless individuals in securing housing. HAF funds may be used towards security deposits, utility payments, ongoing rental assistance, and other forms of rapid re-housing. The program is administered by Advanced Behavioral Health, Inc.
Applications will be submitted online by select DMHAS grant-funded providers and a review committee, including DMHAS staff, will make a determination on all requests. Provider staff who wish to access the online application portal should complete the Login Request Form below and fax to (860) 471-8124 and review the HAF User Manual for instructions on submitting requsts. Applicants and requestors should download and review the HAF Application Packet and/or HAF Utilities Packet in advance as, along with the online request, several forms must be completed and submitted via fax in order for an application to be reviewed.
ABH will notify requestors in writing within 30 days of receipt of a complete application. Please note that documents are not accepted via email and all information must be faxed to (860) 471-8124.
