Intensive Case Management (ICM)

ABH® is the administrative services organization for the Behavioral Health Recovery Program which provides intensive case management services, and outreach and engagement to help individuals with substance abuse disorders throughout the state of Connecticut. This service is open to individuals who are currently active or eligible for Husky D. The goals of the Intensive Case Management Program (ICM) include improving treatment outcomes and decreasing the use of acute treatment services by providing connections to wrap-around supports in the community, including lower levels of treatment, housing, and transportation and other needs in accordance with the client’s recovery plan.

Eastern Region Service Center (ERSC)

ABH® is the administrative services organization for the Eastern Region Service Center (ERSC), which provides case management services, and outreach and engagement services to help individuals with substance abuse disorders in Connecticut’s eastern region. ERSC was initiated in May 2000 as a collaborative effort among eleven mental health and substance abuse service agencies to create solutions together by enhancing consumer access to case management services in the eastern region. ERSC’s goals include improving treatment outcomes and decreasing the use of acute treatment services by integrating the various case management services in place, standardizing case management policies and procedures, and increasing accountability. ERSC is uniquely governed by an Advisory Committee with representatives from all member agencies.

ERSC case managers provide person-centered care and develop recovery plans collaboratively with consumers. They assist consumers with the tools to promote change, enhance well-being, and move toward independent living through gainful employment, housing, and the utilization of social, Twelve Step, and other community supports.

 

Eligibility Requirements

ERSC serves approximately 900 individuals each year. To be eligible, individuals...

  • Must be eighteen years of age or older
  • Must live in the eastern region
  • Must have a substance use, psychiatric, or co-occurring disorder
  • May have no access to medical insurance, either public or private
