POSITION SUMMARY:

The Women’s Recovery Navigator (WRN) provides outreach, engagement, case management, recovery coaching and community connections to treatment and recovery support resources to women, in particular women who are pregnant and/or parenting in the REACH (Recovery, Engagement, Access, Coaching & Healing) Program. The primary goal of the WRN is to help identify, engage, connect and support women in the community as they enter into and sustain recovery from substance use disorders or co-occurring disorders.  Early intervention is provided to support women’s recovery needs and increase connections to community-based treatment. The Women’s Recovery Navigator provides holistic services to maximize the likelihood of client success; increase the rate of client engagement in the recommended services; support successful client treatment outcomes; ensure the client receives quality treatment services in an appropriate treatment setting; and support the client in increasing her recovery capital.  The WRN works from a strengths perspective providing and coordinating a full array of engagement/recovery case management services to women.  This position will represent ABH® and its services in a positive and professional manner and adhere to ABH®’s best practices, guidelines, policies and procedures as established, promoting ABH® as a leader in behavioral healthcare in all interactions and work.

DUTIES AND RESPONSIBILITIES:

  • Assist the woman in developing and promoting healthy connections to children, family, significant others and her community;
  • Work in the community outreaching and engaging women, establishing and maintaining community connections, and facilitating connections between the two; 
  • Help connect clients to services aimed at enhancing their recovery plan while supporting their goals as they develop a safe support network;
  • Provide education, advocacy, coaching and mentorship;
  • Make engagement attempts with clients in various community settings, including clients’ home, shelters, soup kitchens or birthing centers. Efforts may include attempts to contact clients via home visits and /or letters to last known address, phone calls, visits to shelters or soup kitchens, collaboration with DMHAS, other case managers, and visits to treatment agencies;
  • Assist with service planning for non-clinical needs (e.g., housing, employment, childcare referrals);
  • Assistance with arranging transportation to treatment appointments and other community supports;
  • Transportation of the client to essential appointments when no other arrangement is timely or/and practical and such transport can be safely achieved;
  • Assistance in accessing community support services;
  • Assistance in accessing urgent and standard basic needs supports;
  • Face to face and telephonic supportive counseling;
  • Build and maintain referrals and contacts with culturally relevant organizations and individuals such as the following, as needed, to establish and maintain adequate community supports: vocational providers, employers, housing agencies, landlords; health    care providers; probation/parole officers, crisis intervention services, hospital emergency        departments, family members and other natural supports; interpreter services; faith communities (e.g.           Churches, etc.), and self-help groups;
  • Provide a full array of services, including services that guide the course of client treatment through various levels of care. Including, but not limited to the following: Intake appointment scheduling, review of treatment history, arrange and attend case coordination meetings with clients, social workers, and treatment providers to assure that appropriate issues are being addressed in treatment; In person and/or phone contact to acute service providers to participate in treatment and discharge planning with the client and provider.
  • Carry a cell phone during working hours (8:30 a.m. – 5:00 p.m.) and respond to all urgent calls within 30 minutes and all routine calls as soon as possible;
  • Provide transportation to clients, using personal vehicles;
  • Maintain internal communications by notifying supervisors with all relevant client or program-based information;
  • Document all recovery management activities within required time frame and in required format including documentation of all client related activities into electronic database as required by DMHAS;
  • Attend all required staff, supervision and training meetings;
  • Maintain confidentiality of all client protected health information and adhere to all HIPPA related policies and procedures;
  • Demonstrate ethical behavior and cultural sensitivity in all activities involving individuals of diverse backgrounds;
  • Maintains confidentiality of all client protected health information and adheres to all HIPAA related policies and procedures;
  • Perform other tasks/responsibilities as required to support the business operations.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Associate’s Degree in Human Services or related field or equivalent combination of 4+ years in recovery & a minimum of 4+ years of sobriety;
  • Self-identified woman in recovery (substance use or co-occurring disorder) who has maintained a minimum of 2 years of sobriety;
  • Willingness to use lived experience to help others
  • Recovery-oriented training certification or will complete (within 2 months of hire);
  • Working knowledge of the recovery community, including 12-step facilitation
  • Demonstrates extensive knowledge of recovery-based supports, both traditional and non- traditional, and demonstrates the ability to integrate these resources into treatment;
  • Working understanding of mental health disorders and how these diagnoses may impact recovery;
  • Demonstrated knowledge and understanding of the role and impact of trauma and gender on all aspects of outreach, engagement, treatment and recovery  
  • Ability to maintain an unbiased, working knowledge of methadone and other medication assisted treatment formulations as a tool to support recovery; 
  • Working knowledge of women’s issues and is comfortable discussing topics such as sexual and reproductive health including birth control and abortion, intimate partner violence, pregnancy, etc. 
  • Demonstrated ability to work collaboratively with clients, their family, community partners and agency professionals;
  • Demonstrated ability to clearly document services provided
  • Experience working with women in recovery;

KNOWLEDGE/SKILLS/ABILITIES:

  • Demonstrated knowledge of engagement services, substance abuse, mental health services, parenting, women’s issues, and community resources;
  • Bilingual skills preferred (English/Spanish);
  • Must have a valid driver’s license and reliable transportation and be willing to transport clients in personal vehicle;
  • Proof of car insurance verifying minimum insurance coverage of $100,000/$300,000/$100,000;
  • Strong attention to detail; ability to work on multiple tasks and meet deadlines;
  • Excellent PC skills with demonstrated experience using Microsoft Office Package (MS Word, Excel, Power Point, Access, Outlook); internet;
  • Strong written and verbal communication skills required.
  • Attends annual Conflict of Interest training.
